IEHP Long-Term Services and Supports (LTSS)

Trusted Care for Seniors & People with Disabilities

IEHP’s Long-Term Services and Supports help IEHP Members who cannot live alone without help, or who have a disability or other health issues. We can connect you to these services:  

In-Home Supportive Services (IHSS)

(for those older than 65, disabled or blind)
Members can apply through the county for IHSS. They can hire caregivers to help with: 
• Cooking meals and cleaning up
• Personal care
• Laundry and housekeeping

Multipurpose Senior Services Program (MSSP)

(for Members at least 65 years old with a disability) 
Covered benefits range from: 
• Case Management
• Minor home modification
• Emergency response system

Community Based Adult Services (CBAS)

CBAS are adult day healthcare centers – for Members 18+ with health issues, making it hard to care for themselves. They must attend 2 – 5 days to get services such as: 
  • Intermittent nursing
  • Ongoing monitoring
  • Social services
  • Therapeutic services 
  • - physical therapy
  • - occupational therapy
  • - speech therapy
  • Lunch & snacks
  • And other services

Skilled Nursing Facility (SNF) 

The IEHP Transition of Care Team helps Members return to their home after a stay at a Skilled Nursing Facility. View this video to see the valuable service available to our members. Click the image below to watch the video!

You may qualify for SNF services if you have a physical disability and need a high level of care. SNF services must be prescribed by your Doctor and given in a licensed SNF. Covered benefits:
    • Skilled nursing care on a 24 hour per day basis
    • Care management
    • Bed and board (daily meals)
    • X-ray and laboratory
    • Physical, speech and occupational therapy
    • Prescribed medicine
    • Medical supplies, appliances, and equipment normally given by the SNF
