An Advanced Care Directive is a legal form that allows you to choose someone to communicate your health care decisions when you are not able to; like in the case of a car accident or a medical emergency. The “My Life, My Choice” is a 1.5 hours class that helps you to better understand that legal form, when you will use it, how to fill it out, and will provide you the actual form.
